14Report Amendments

Owner
Admin for typographical changes; clinician for anything factual or clinical
Trigger
A patient requests a change to their report.

Purpose

Handling requested changes to reports and letters, and recording how long they take to action.

Procedure

  1. Log the amendment request on the amendments tab.
  2. Assess what is being asked for.
  3. If it is a typographical error, admin makes the change directly.
  4. If it is a factual inaccuracy or touches clinical content, the request goes to the clinician who wrote the report. Admin does not amend clinical content.
  5. Action the amendment and record the time taken.
  6. Return the amended report to the patient and update the relevant report tab (09, 10 or 11).

Fields recorded

NHS number · name · assessment type · doctor or practitioner · nature of amendment · date amendment received · time taken to action · details

Notes

  • The split is the important rule here: typo goes to admin, factual inaccuracy goes to the clinician.

Open points

  • Whether the time-to-action metric measures admin processing time or total turnaround including clinician review.
