14Report Amendments
- Owner
- Admin for typographical changes; clinician for anything factual or clinical
- Trigger
- A patient requests a change to their report.
Purpose
Handling requested changes to reports and letters, and recording how long they take to action.
Procedure
- Log the amendment request on the amendments tab.
- Assess what is being asked for.
- If it is a typographical error, admin makes the change directly.
- If it is a factual inaccuracy or touches clinical content, the request goes to the clinician who wrote the report. Admin does not amend clinical content.
- Action the amendment and record the time taken.
- Return the amended report to the patient and update the relevant report tab (09, 10 or 11).

Notes
- The split is the important rule here: typo goes to admin, factual inaccuracy goes to the clinician.
