10Autism Assessment Report

Trigger
An autism assessment has taken place.

Purpose

Tracking an autism report from assessment through drafting, amendments and issue to the GP.

Procedure

  1. Add the patient to the autism reports tab, recording both the lead practitioner and the second practitioner.
  2. Complete the 1st draft of the report. Approval is the lead practitioner's responsibility - the second practitioner does not need to sign off before the draft goes out.
  3. Report is formatted by admin, as with ADHD.
  4. Record the diagnostic outcome.
  5. If no diagnosis was given, the patient is discharged (C5) and no 6-week follow-up is offered.
  6. Send the 1st draft to the patient and record the date. Send resources and record the date.
  7. If amendments come back, record the date received, handle under 14, and record the date the amendments were completed and sent to the patient.
  8. Send the final report to the GP and record the date.
  9. Set Pathway complete = Yes and filter out. If a diagnosis was given, the patient moves to the 6-week follow-up pathway (13).

Notes

  • Autism assessments require two practitioners, but only the lead practitioner approves the report.
