09ADHD Assessment Report

Trigger
An ADHD assessment has taken place.

Purpose

Tracking an ADHD report from the end of the assessment to the final copy landing with the GP, and measuring how long each stage takes.

Procedure

  1. Add the patient to the ADHD reports tab and assign the report to a named member of admin.
  2. Format the report. Mark formatted.
  3. Send for clinician approval. Mark sent for approval, then checked once returned.
  4. Record whether the patient was given a diagnosis.
  5. If no diagnosis was given: cancel the post-diagnostic appointment, move the patient to the post-diagnostic tab (12), and discharge (C5).
  6. Send the report to the patient and record the date. Send resources and record the date.
  7. If amendments come back, record the date received and handle under 14. Record the date the amended report was sent back to the patient for approval.
  8. Send the final report to the GP and record the date.
  9. Set Pathway complete = Yes and filter out.

  • The days-between metrics are calculated columns and are what the ICB reporting draws on (28).
