11Full Psychiatric Assessment Report

A full psychiatric assessment has taken place.

Purpose

As per the autism report process, with an additional consent step before anything is sent to the GP.

Procedure

  1. Follow the autism report process (10) for drafting, formatting, approval, issue to the patient, resources and amendments.
  2. Record whether the patient consents to the report being sent to their GP. Full psychiatric assessments are private patients only, so GP sharing is not automatic.
  3. If consent is given, send the report to the GP and record the date.
  4. If consent is refused, do not send. Where the clinical team judges there is a risk the GP needs to know about, the decision to inform the GP anyway is taken at the Daily Brief or MDT (C4).
  5. Set Pathway complete = Yes and filter out.

Notes

  • This pathway applies to private patients.
  • Consent can be overridden on clinical risk grounds, but only through the Daily Brief or MDT - never by admin.
